Charlotte Ann Polk, 70, of Denton, passed away Sunday, March 6, 2016 in Denton. She was born June 5, 1945 in Dallas to J.T. and Effie Lewellen Smith. She married Kim Tyman Polk on October 3, 1970 in Dallas County. He preceded her in death on September 16, 2009. She was a resident at Senior Care Health and Rehab in Denton and shared many good times with both staff and other residents.

A graveside service will be Wednesday, March 9, 2016 at 2:00 P.M. at Roselawn Memorial Park in Denton with Rev. Max Brennan officiating.

She is survived by her daughters, Kathy Efflandt and husband, Randy, Deborah Pittsinger and husband Terry; grandchildren and extended family, Jacklyn Platt and husband Ronnie, Clint Moon, Austin Pittsinger, Monica Moon and husband Colby; and 5 great grandchildren.
